UBC Quality Management Complaint Form
UBC is aware that sometimes a parent, peer, or individual may have concerns about the care provided by a Licensed Midwife. It’s important to uphold the safety and integrity of midwifery by addressing and investigating these concerns.
To file a complaint, download the attached form and follow the instructions on the form. It will be reviewed by a committee to determine if the case requires further investigation. Every response and complaint is viewed as serious and treated with sincerity.

The United Birth Coalition was formed to achieve the mission of increasing access to midwifery care for mothers and babies across the state of Ohio and beyond. We have been working at a grassroots level ever since, changing the legislation and building a network of providers to make it easier for mothers and their babies to get the care they need and deserve through pregnancy, birth, and beyond.
FAQs
How do my monetary contributions help create safer birth options in Ohio and Indiana?
Financial contributions made to United Birth Coalition support legislative and lobbying efforts to create laws that will ensure midwives are properly licensed in Ohio, freestanding birth centers are supported, and mothers and newborns have appropriate access to midwifery care before, during, and after birth.
Are my donations tax deductible?
Due to United Birth Coalition’s status as a 501(c)4 non-profit organization, donations and membership dues are not tax deductible as a charitable contribution or as a business expense under IRC Section 162(e). Your contribution will not be earmarked for any purpose, and will generally support our work to increase birth options, access to midwives, and to make Ohio a safer place to have a baby.
I’m a parent, what is the best way I can participate in UBC?
You can join UBC as a Parent Member, meaning you will have access to member-only resources and events. You are invited to make a one-time or recurring donation to UBC if membership does not fit your needs and/or you can volunteer your time to help out at our events when needed. We are also always looking for more help on our board! If you have certain skills or interests you feel we could benefit from, submit a Board Member request.
